Disaster Check-Out Procedures
Procedure for checking students out of school during a disaster:
Parent(s) most bring a note to the school with valid picture identification (drivers license). The note should contain the following information:
- Date
- Student's name and homeroom teacher
- Time of day which the student was picked-up
- Parent's signature
NOTE: Students will ONLY be release to authorized family members whose names appear in SMUSD Health and Emergency Information card. If you would like to add additional names please contact the HMS office.
